Proposed Conditions

1. Adherence to the proposed Good Neighbor Policy.
2. Controlled access including fencing cameras, berms and natural buffers at the quarry will be provided as needed. Gates to the site will be locked during non-business hours.
3. Quarry operations will not operate on Sunday or major holidays.
4. Blasting will be limited to the hours between 10:00 a.m. and 4 p.m., Monday through Friday, with no blasting on weekends or major holidays.
5. Putnam County Stone will ensure noise levels at the property line will not exceed 70 dBA Leq during the hours of operation. The decibels shall be measured using the A-weighting, slow-response network by a noise level meter meeting, at minimum, Type II standards.
6. Putnam County Stone agrees to maintain ground vibration peak particle velocity from blasting activities at levels <1.0 in/sec measured at the current nearest structure (~2,225’). Putnam County Stone will utilize 5 seismographs, distributed around the periphery of the property and/or on neighbors’ properties, as permitted, to document vibration and air overpressure levels for each blast. Results will be shared with the county, as requested.
7. Putnam County Stone agrees to maintain air overpressure from blasting activities at levels <120 dBL measured at the current nearest structure (~2,225’).
8. Putnam County Stone will comply with all US and state blasting regulations. Records and procedures will be made available to Putnam County upon request.
9. Putnam County Stone will employ all necessary, modern dust control practices to maintain daily average particulate matter concentration levels measured as PM10 below 150 micrograms/m³ at the property line in accordance with NAAQS standards, as demonstrated using GA EPD approved procedures and US EPA’s approved AERMOD air dispersion model. Putnam County Stone will also install dust monitors around the mining site to monitor for potential fugitive dust emissions. Results from the dust monitors will be shared with the county, as requested.
10. Putnam County Stone will make reasonable efforts for trucks leaving the site to minimize emissions of dust and debris.
11. All haulers will ensure compliance with current DOT Inspection requirements.
12. Putnam County Stone will install monitoring wells on the property prior to commencement of mining activities and will maintain the monitoring wells throughout the life of the operation. Water levels in each well will be recorded daily and collected quarterly. Water quality data will be collected twice per year and analyzed by a third-party lab. Results from both water level measurements and water quality test will be shared with the county, as requested.
13. Putnam County Stone will maintain a Quarry Neighbor Water Well Policy to ensure the sustainability of neighboring residential water wells.
14. Putnam County Stone will provide a tire wash and paved entrance/exit. The paved entrance will include space for inbound truck staging off of public roads.
15. Putnam County Stone’s exit onto Dennis Station Road will force trucks to turn right only heading north. In addition, Putnam County Stone will pay to install an acceleration lane heading north on Dennis Station Road.
16. Putnam County Stone commits to widening Dennis Station Road near the driveway intersection to help alleviate current and future traffic congestion. These additional lanes will include an additional right turn lane/queue storage lane for the trucks entering the Interfor paper mill, and one northbound acceleration lane for the trucks exiting the PCS site to merge with the northbound traffic on Dennis Station Road heading toward US 441.
17. Storm water management, threatened and endangered species, wetland impacts, cultural resource mitigation, air permitting, mine permitting, and water discharge permitting, are regulated by the state and or the US government. Putnam County Stone will comply with all applicable regulations and permits.
18. A performance guarantee or reclamation bond shall be obtained in accordance with GA EPD Regulations to ensure that the provisions of the Reclamation Plan are met. Should GA EPD at some point in the future not require a performance guarantee or reclamation bond, a similar replacement bond will be placed to the benefit of the county.
19. Putnam County Stone is committed to protecting all area rivers and lakes from sediments and contaminants. All water discharged from disturbed areas of the site will be discharged through NPDES compliant BMP water sediment basins in compliance with all NPDES permits and their prescribed regulations and procedures for stormwater and process water which include the following water quality parameters.

20. A $25/hour minimum wage will be implemented for all full-time employees of Putnam County Stone.
21. Uses depicted on the site plan will be adhered to in the locations shown and the only industrial use on the property will be mining and related uses.
22. The adherence to the conditions listed and agreed to above are understood to be required to maintain county permission for the operation of any construction aggregate mining and processing facility on this property now and at all times in the future.
